Inusual ictal paralysis due to inhibitory motor seizures mimicking a cerebral transient ischemic attack: two cases and a brief review of epileptic ictal paralysis Brief historical development of inibitory motor seizures

چکیده
Negative phenomena can occur with seizures, but inhibitory motor seizures are rare and may lead to misdiagno sis. Two adults with focal inhibitory motor seizure (ictal paralysis) have been evaluated, and taken together with the clinical history, the results from these investigations suggest focal inhibitory seizures as the underlying etiology. The mechanisms of negative motor phenomena during a seizure are not well understood, but ictal negative motor fenomena have been reported in a frontal, central, or parietal epileptogenic focus. Inhibitory motor seizures must be differentiated from transient ischemic attacks. A brief review of some of the possible underlying physiological mechanisms of these unusual features is of interest.

Ictal paresis associated to PLEDS in two children: A video-EEG study
Ictal paresis (IP) is a rare negative motor phenomenon presenting challenging differential diagnostic problems with transient ischemic attacks, post-ictal paralysis, migraine and psychogenic paralysis. Video-EEG undoubtedly represents the essential mean for a proper diagnosis. Periodic lateralised epileptiform discharges (PLEDs) are a distinctive EEG pattern, consisting of periodic spike or sha...

Recognition of a transient, focal neurologic dysfunction after a seizure is important when evaluating patients with idiopathic epilepsy. Todd's palsy, a transient focal weakness after a seizure, is a highly accurate clinical sign indicative of a contralateral, cerebral epileptic focus. In contrast, a transient, lateralized hyperkinetic motor behavior from a contralateral, hemispheric ictal focu...
متن کاملTransient paralytic attacks of obscure nature: the question of non-convulsive seizure paralysis.
Eleven patients with transient paralytic attacks of obscure nature are described. Paralysis could involve face or leg alone, face and hand, or face, arm and leg. The duration varied from two minutes to one day. Four patients had brain tumors, six probably had brain infarcts, and one a degenerative process. The differential diagnosis included TIAs, migraine accompaniments, and seizures. In the a...
متن کاملTransient anosognosia for episodic hemiparesis: a singular manifestation of TIAs and epileptic seizures.
Anosognosia is a well-known manifestation of non-dominant parietal lobe lesions and typically lasts a few days. That anosognosia may last only a few minutes to a few hours, as observed in six patients, has not been reported. In five patients, transient anosognosia for equally brief left-sided hemiparesis was a manifestation of transient ischemic attacks (TIAs). In the sixth patient, anosognosia...
متن کاملPrediction of Epileptic Seizures in Patients with Temporal Lobe Epilepsy (TLE) based on Cepstrum analysis and AR model of EEG signal
Epilepsy is a chronic disorder of brain function caused by abnormal and excessive electrical neurons discharge in the brain. Seizures cause disturbances in consciousness that occur without prior notice, so their prediction ability, based on EEG data, can reduce stress and improve quality of life. An epileptic patient EEG data consists of five parts: Ictal, Inter-Ictal, pre-Ictal, Post-Ictal, an...
